Your local United Way will either have some on hand or will be able to get some for you. Additionally, you can print them directly off of the FamilyWize website.

On the FamilyWize website, there is a tab to look up participating pharmacies in your area. This is also a great place to check for any changes in acceptance by pharmacies.
